Use this script when calling participants in your Incoming Referral list.
Hi [participant name], I'm calling from the Long Beach Health Department. How are you doing today?
[respond accordingly]
On [date of referral] you visited the [referring program] and said that you were interested in learning more about [program name].
I’m calling to follow-up and see how our program can assist you. Is now a good time to chat?
[If participant says no, ask if they'd like you to call back or email with more information]
[If participant says yes, proceed providing additional information about the program and connecting them to services].
