Adding a participant, step by step:
Any time a participant receives a service from your program they must be added to the ISC so the Health Department has a record of the visit. If the participant has been seen in the last 90 days, you will not need to do health screening.
1. Click on the “Participant” tab in the left menu
2. Select “Add Record”
3. Enter the name and DOB for the new or returning participant & click Submit. If there is an existing record for the participant (perhaps they were seen at your program or another DHHS program) it will appear here.
4. If your participant was found, select their profile and proceed. If your participant was not found, click “add a new participant.”
5. Complete the intake fields and click submit.
6. If the participant is due for a new health screening, click “Go to Health Screening” and complete the questionnaire with the participant.
7. If the participant is due for a new health screening, click “Go to Health Screening” and complete the questionnaire with the participant. Close the browser tab with the Health Screening survey and return to the tab with the ISC.
8. Click “Review Eligibility” and discuss what programs the participant may find beneficial and ask what referrals they are interested in.
9. Send the referrals and add a note to provide the receiver context on the conversation you’ve had with the participant.
