Asthma
The asthma program is a well-established asthma intervention and case management program. One-on-one education program where adults can learn how to manage asthma, identify asthma triggers in the home, how to speak to their health care provider and how to use their asthma medications.
Who the program serves: Adults and Seniors
Eligibility Criteria Summary: The Asthma program can help participants determine eligibility. Participants must be an adult with asthma who resides in the eligibility zones.

Lead
The lead program aims to control and prevent housing/environmental conditions which result in childhood lead poisoning. This is accomplished by providing education to the public; coordinating with City agencies to identify and rehabilitate high-risk affordable housing; investigating and early response to families of lead-poisoned children; enforcement of applicable laws; and response to community complaints.
Who the program serves: Families with young children who are low-income and live in older, deteriorated housing.
Eligibility Criteria Summary: The lead program will help tenants determine eligibility. Tenants must meet HUD income requirements and have children under 6 living in the home.

Healthcare Access (Medi-Cal Team)
The Healthcare Access program provides outreach, education, enrollment, and forms assistance on the available health programs, such Medi-Cal and Covered CA. In addition, we offer advocacy, troubleshooting and program navigation.
Who the program serves: anyone who is uninsured
Eligibility Criteria Summary: The Healthcare Access Program will help participants with the complex eligibility calculations. As an overview, eligibility is based on household income, family size and immigration status.

Family Planning
Provides comprehensive family planning services to uninsured and underinsured
women and men of childbearing age. Services aim to promote reproductive health and to reduce unplanned pregnancy by lowering the barriers that many women and men face in obtaining family planning services.
Who the program serves: Uninsured and underinsured women and men
Eligibility Criteria Summary: Services are available for FPACT eligible men and women at no cost. Some insurance plans are also accepted and sliding/reduced fees (based on income) are available for uninsured and underinsured.

Immunizations / Travel
Low-cost routinely recommended immunizations to uninsured and underinsured children and adults. Assessment, education, and immunizations to clients traveling to countries where immunizations are required or recommended.
Who the program serves: Uninsured and underinsured children and adults
Eligibility Criteria Summary: Medi-Cal eligible patients can receive vaccines under the Vaccine for Children (VFC) Program or Vaccine for Adult Program (VFA). Travel Vaccines are private pay and costs are subject to change based on actual cost to LBDHHS.

Multi-Service Center
The MSC is an access point to services for people experiencing homelessness.
At the center, individuals can access basic services (showers, mail, food, clothing, etc.) and be linked to more comprehensive supportive services (short or long term housing, employment services, mental health resources, etc.).

Tuberculosis
Provides expert TB medical care to residents who are suspected or known to have TB disease. We also provide testing for exposed TB contacts. The clinic does perform routine TB screening, such as those for employment or school admission.

Eligibility Criteria Summary: TB medical treatment services through our clinic are free to patients. Insurance, Medi-Cal or other coverage is billed if available.

HIV Care & Coordination
The HCC Clinic provides medical services, health education, case management, treatment advocacy, support and direct linkage to outside services when needed.
Who the program serves: Uninsured and underinsured women and men
Eligibility Criteria Summary: Services are available for the uninsured at low or no cost and some insurance plans are accepted.

Sexual Health (STD Clinic)
Sexual Health Clinic offers comprehensive sexual health services which including STD testing treatment for: gonorrhea, chlamydia, herpes, trichomoniasis, syphilis, genital warts, HIV, HPV, and Hepatitis B & C.
Pre-exposure Prophylaxis (PrEP) prevention prescription medication for people who do not have HIV, but who are at substantial risk for contracting HIV. PrEP services include: PrEP medical care, mediation management, and healthcare navigation.
Who the program serves: Uninsured and underinsured women and men
Eligibility Criteria Summary: We accept the following insurance plans: Medi-Cal, Health Net, LA Care, Medicare (Non-HMO), Most PPO’s (out-of-network only). Sliding/reduced fees (based on income) for comprehensive STD testing & treatment and Biomedical HIV Prevention Services are available for the uninsured, underinsured, or if you would prefer not to use your insurance due to confidentiality concerns.

Nurse Family Partnership
NFP is an evidence-based voluntary program that offers a free nurse mentor to provide:
- Pregnancy education and support
- Health promotion for optimum child development
- Promote Infant and child injury prevention
- Support skill building for self-sufficiency & confidence with motherhood

Fundamentals of Fatherhood
The program works to improve father-child bonding, co-parenting, economic stability and mobility. It offers Life coaching, the Nurturing Fathers parenting program in 10 weekly sessions and employment development. Fathers that can attend program services and activities in Long Beach are welcomed to enroll. All services through Life Coaching and Fundamental of Fatherhood are FREE.

Family Preservation
The Family Preservation Program provides family support to families referred from the Los Angeles County Department of Children and Family Services (DCFS) and the Los Angeles County Probation Department. Family Preservation is designed to strengthen and preserve families whose children are at risk of placement in out-of-home care. Services include weekly in-home visitation, parenting classes, counseling, job training services, mental health, educational and extensive linkages to community services. To participate, families must be referred directly by the Los Angeles County Department of Children and Family Services (DCFS) or the Probation Department. Families may receive services for six months to one year. The Family Preservation Program serves approximately 200 families and 600 children/youths annually.

Resource Connections
The Long Beach Resource Line supports all individuals and families in Long Beach who need support finding and accessing resources. Residents can call 562-570-INFO (4636), select Option 5 to speak with a resource navigator. Staff are trained to connect residents with the appropriate resources and local services that best meet their needs. Service types that can be requested through this hotline may include health care, mental health, food access, housing and shelter, financial support, transportation, utilities, legal aid, children and family services, older adult services, and support for veterans, members of the LGBTQ+ community and individuals who are undocumented. WIC
The WIC Program is a Supplemental Nutrition Program that provides monthly healthy food benefits, nutrition education, breastfeeding support, and referrals to medical and social services in the community.
Who the program serves: WIC serves pregnant and postpartum women, infants and children up to age 5.

Public Health Field Nursing
Public Health Nurses provide services for Maternal, Child and Adolescent Health. A Public Health Nurse can provide a home visit for health evaluations, case management, teaching, and referrals to community resources. We can provide services for a first time expectant mother with a high risk pregnancy, premature infants, congenital conditions, developmental delay or children with a communicable disease.

Healthy Active Long Beach: Learn basic nutrition information, healthy cooking and snack ideas, tips for clients, and how to keep you and your family healthy
GreenlightLB: serves those who live, work, and play in Long Beach through being an unbiased and trusted resource for health and safety information related to cannabis, and by ensuring that everyone can make informed decisions about cannabis use.
Tobacco Education: works to build a healthier community by reducing the use of tobacco and the exposure to secondhand smoke in the City of Long Beach.
Walk and Roll Long Beach: providing pedestrian and bicycle education and community events.

Public Health Emergency Response Operations
The Public Health Emergency Response Operations Program is the mobile in-home COVID-19 vaccination team. This service is for anyone that is unable to go to any of the vaccination clinics throughout the city due to mobility issues, lack of transportation or any other reasons. The mobile vaccination team will go out to facilities like nursing homes, senior housing, and conduct community pop-ups.
